Incoming Fayette Student Forms

In order to expedite the process, please bring your completed forms with you on your scheduled OAR (Orientation, Advising & Registration) Day. Forms can be downloaded from myUIU below. 

Residence Hall Students:

• Housing Application (must be completed online) 

Off-Campus Students:

• Off-Campus/Commuter Request Form (must be completed online) 

Parking on Campus:

Registration forms and payment can be found in myUIU.  Login with your credentials, click on the tab UNIVERSITYWIDE and click on the folder titled FORMS.  The link can be found on the FORMS page.   

Important Documentation

• Peacock Intake Form (Required for all new students. Must be completed online & log in to myUIU required.)

Student Athletes:

• Student athlete forms can be downloaded from the UIU athletics website
