Test Accommodation Form

A common accommodation for students with disabilities is taking exams in a quiet location. UIUDS administers all exams in the testing room located within the Office of Student Development, unless the room is already occupied, in which case another location will be used in OSD. If instructors wish to have UIUDS proctor exams, a Test Accommodation Form must be completed cooperatively with the student and submitted to the Coordinator at least 48 hours in advance of the exam. Although each student who receives the accommodation of testing in a quiet location will receive a copy of this form, it can also be found by clicking here.
