Background Check Form Instructions

As part of the admissions process, you will need to complete a background check and provide proof of your health record.  This process needs to be completed through CertifiedBackground.com.  Please click on the appropriate link below to view the instructions from CertifiedBackground.com on how to complete this process.  Please note that these requirements are mandatory and allow Upper Iowa University's Nursing Program to comply with Arizona and Iowa Board of Nursing regulations as required by law
